File a Complaint

The Office of Financial Regulation (OFR) welcomes complaints from concerned citizens because they may be the only way we become aware of fraud or unfair practices in the financial industry. Your complaint will be recorded in a database and analyzed for a pattern of wrongdoing that may result in a formal investigation or action to protect the public.

Information We Need to Process Your Complaint

  • Summary of the complaint describing its nature, name of the firm or individual(s) involved, and what was said or done resulting in your need to file a complaint.
  • Copies of related documents: provide copies of all documents that support your complaint. Examples are contracts, account statements, disclosure documents, cancelled checks or money wires, sales materials and advertisements. Filing online allows you to upload electronic documents.
  • Online Services Account Registration: In order to file your complaint, you will need to register electronically through our Online Services. A valid email address is required.
  • Username and Password: Once registered, your username and password will allow you to quickly check the status of your complaint at any time, upload additional documentation electronically, or file a new complaint without having to register again.

Note: Consumers are encouraged to file their complaints online. If you are unable to file online, please download and mail a completed paper form. Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to the Office. Instead, contact this office by phone or in writing. Communications with this office, regardless of the form or means of transmission, are subject to disclosure under Florida's public records law.
